By providing your phone number, you are enrolling in the Health for Her Text Alerts Messaging program and agreeing to the following Terms and Conditions: When you enroll, you certify that A) you are the account holder and consent to enroll or, if you are not the account holder, you have the account holder’s consent to enroll and B) you are age 18 or older. The Health for Her Text Alerts Messaging program is a subscription service. There is no charge from Health for Her for this service, but YOUR SERVICE PROVIDER’S MESSAGE AND DATA RATES MAY APPLY. The texts will be sent via automated technology. You enroll to receive automated messages. Participation in the Health for Her Text Alerts Messaging program is not required for services. You may receive multiple messages related to the status of your services. These messages may include, without limitation, notifications when your appointment is scheduled, requests to confirm your appointment, or reminders about your appointment. You understand and agree that these messages may include limited personal information about you and your services, and whoever has access to the mobile phone or carrier account will also be able to see this information. You acknowledge and understand that messages are sent without encryption, which presents some security risk. Unencrypted messages could be viewed by a third party. You may opt out of all SMS/MMS communication at any time by texting STOP to 54753 or calling (833) 766-5404. For help, text HELP to 54753 or call (833) 766-5404 to adjust your preferences.
